Referrals to Gold Star Hospice Care are welcomed from hospitals, healthcare providers, or by word of mouth for patients diagnosed with a terminal illness.
Our dedicated Medical Director can admit patients directly into our program.
Gold Star Hospice accepts all patients regardless of race, religion, age, gender, sexual orientation, or national origin.

Admission Criteria
- The patient has a serious illness or advanced condition and is seeking comfort-focused care.
- Life expectancy is estimated by a physician to be six months or less if the illness follows its typical course.
- The patient and/or their family understands and prefers a focus on quality of life over curative treatments.
- The patient has a physician willing to coordinate hospice care or is open to our Medical Director providing oversight.
- The patient lives in a private home, care facility, or other residence within our service area.
Benefit Eligibility
Anyone with Medicare, Medi-Cal, or other qualifying health insurance can receive hospice benefits if their primary doctor certifies they have a life expectancy of six months or less (if the illness runs its normal course).

Choosing Hospice Care
Hospice care is a choice made by patients and their families when comfort and quality of life become the main focus. During admission, you and your family will review and sign consent forms, ensuring you understand and agree to hospice services.
Our hospice team will regularly check on your condition. If your health stabilizes or improves, we will work with you and your family on a safe discharge plan. If you need hospice services again in the future, you can be re-admitted as long as you continue to meet eligibility guidelines.
